Financial

Our practice accepts cash, check, and major credit cards. Dr. Kuchar is a provider of almost all insurance plans. Please call our office to check if we participate with your insurance if you don't see Dr. Kuchar in your plan.

Cancellation

If you would like to cancel an appointment, we ask that you provide us with at least 24 hours notice. We kindly ask for this advance notice so that we may offer this appointment to another patient. A fee may be charged for a no-show or late cancellation.
